From hypothesis and instinct to a live action plan

CHCP provide adult community nursing services across a large rural footprint and wanted to identify practical ways to ease operational pressure.

Nursing staff from Hull North, Beverley, Driffield and Bridlington carried the Heim Companion App over two months.

Heim combined GPS, SystmOne data, shift shadowing and allocation observations to map travel, visits and administrative pressure.

The pilot mapped 33,109 miles of community nursing travel and showed that around a third of every tracked hour was spent in motion.

The findings helped CHCP close the pilot with a clear, owned, board-backed plan.

Four working groups are now driving change across IT and digital enablement, data and capacity, workforce pressure and staff safety, and cluster reviews.
