Manchester NHS Foundation Trust engaged Heim to support District Nurses delivering complex care across North, East, Central and South Trafford.

Teams were managing congested inner-city patches, rural routes, unplanned care, manual deferral tracking and rota building across EMIS, spreadsheets and paper processes.

72 staff members carried the Heim Companion App while the Heim clinical team shadowed shifts, sat in on allocation and combined observations with GPS and EMIS data.

The pilot captured 8,435 finished visits, 3,890 miles and 239 hours of drive time.

The work showed deferred care rates between 12-29% of planned visits, visit durations routinely overestimated by just under 30%, and routing opportunities that could cut distance by around a quarter.

Heim also identified the potential to save up to four hours a day for senior Band 7 nurses coordinating care.
